The Social Service Specialist/Social Worker provides intensive social work services to families with complex post pandemic situations requiring individualized, comprehensive, culturally competent assessment, service planning, referral/linkage, monitoring, and advocacy.
A knowledge base of persistent mental illness, socio economic situation, post COVID-19 sequelae, and related community resources is preferred. This position is a part of an interdisciplinary team assisting individuals in connecting resources for community members experiencing socio economic impact from COVID 19, medical and mental health disparities in the targeted community. Support community members who meet the established criteria in getting transportation to vaccination sites by providing bus tokens, ride share reimbursement, etc.
This is a part-time position (0.5 FTE) which may require working on some Saturdays and Sundays; generally scheduled within Monday thru Friday (8-4:30pm).

Job Discription
• Provide medical and mental health targeted case management to individuals at risk to mental illness.
• Engage individuals to find resources, and goals to ensure health, safety, and access to the best level of care in the community.
• Work closely with a wide range of community, County and State partners to assess, intervene and resolve a variety of time-sensitive financial, benefit, clinical, and provider barriers to facilitate individuals needs.
• Assess the needs with a person/family-centered and culturally competent approach to determine a plan of action for their problems.
• Work collaboratively with the interdisciplinary team (COVID Community Coordinators other staff) to find resources for community members in need.

Job Requirements:
• Bachelor's degree in social work, psychology, sociology, human services, or equivalent field. • Preferably licensed as a Social Worker by the Minnesota Board of Social Work. • Culturally competent
• Bilingual in Amharic and English
• Has a valid driver's license and daily access to reliable transportation to travel primarily within/outside of the metro area to meet with clients in their residential settings and/or perform authorized off-site work duties.
• Excellent communication and organizational skills.
• Preferred Experience: Working with adult individuals with complex medical, mental and socio-economic needs and/or lived experience with the target community.
• Preferred Experience: Providing social services to people from diverse cultures or communities.
• Preferred Experience: Community-based case management.
• Eligible to work in the United States
